Forms

  • Participant Registration Form (2009-10 Family Book Club Registration Form) Each participant completes one of these forms once per year when they become part of the Book Club.  Trainers must keep these forms on file at their Book Club site for three years, then they may be destroyed/shredded.

Use this form to plan your monthly Learning Triangle presentation.

Use this form to plan what you will teach each month during your Book Club session.

    • 5th of each month Book Club report from previous month due
    3rd week of each month package with book and possibly other materials will be sent to you
    December 30 return DUE DATE for books not distributed in September, October, November, & December
    late March request participation certificates for participants who have attended Book Club sessions
    March 30 return DUE DATE for books not distributed in January, February, & March
    April or May administer End-of-Year Evaluation surveys and then return to IPTV
    June 15 return DUE DATE for books not distributed in April, May, & June
